In this post on how not to buy software, I stated that software development and implementations often fail due to what I called self inflicted wounds.  Meaning that no software vendor can make up for an SME organizations errors or omissions.

Many of those same warnings are in this quote by Gary Harpst, author of; Six Disciplines Execution Revolution

He puts it even clearer, not just software implementations or projects; but why companies fail.

Failure to understand the customer

Failure to understand the resources, time required to execute the strategy

Failure to obtain senior leader commitment

Failure to execute the plan
